Doesn't seem to be any official documentation. It's suppose to be a shop card that can only be used at Costco or T-Mobile. It automatically arrives about 2 weeks after the 90 day return period. I would assume the 90 day return period starts when the new phone is delivered.

When I was purchasing new phone on new line 5goPlus plus additional 2 new lines plus internet upgrade I was presented with costco/t mobile promo which stated that I would get costco card with credits of $150 per line plus $75 for internet upgrade. So $450 +$75= $525 in total.

Had rep do the port from Verizon, submit my $150 Costco Cash card promotion, and setup my free Netflix account. Later I found ot there was a $200 virtual Mastercard offer from T-mobile, for porting from Verizon, that the rep didn't tell me about. I called T-mobile rebates, told the rep my story, and she submitted the $200 promo request for me.

I just switched from Verizon to T-Mobile and did it through Costco. The process was seamless. I brought two of my own devices and got 3 lines (I don't even use the third line) for $120/month because of a hookup code and got $225 in Costco shop cards and a prepaid Visa for $200 that can be used at T-Mobile or Costco. I couldn't be happier.

Yeah it is for real. I got my Samsung S21 Ultra at Costco, but my trade in was a Samsung S8+ so only got $450 credit. This was done within the month after I got. $45 credit to T-mobile account and then a $405 virtual visa card was sent via email. Which when calling the company found out you can do a balance transfer to your checking account.
